Care Home Survey

NHS Nottingham and Nottinghamshire are in the process of reviewing the Care Homes Team (NHS) services, with the aim of creating a single model to replace the four different offers currently in practice countywide. As a resident, carer or family member of a resident who may have received these services, we are really keen to hear your thoughts about your experiences.  

This survey is regarding the service received from the care home service team only. Across Nottingham and Nottinghamshire, these services have different names.

A summary of the different names are below: 
– Mid Nottinghamshire Team
– Bassetlaw: Bassetlaw Care Homes Team 
– Nottingham City: Integrated Care Homes Team, Specialist Nurses 
– South Nottinghamshire: Delivered from the GP Practice’s: Enhanced Health in Care Homes Teams/ Care home support
