NHS Health Check Patient Surveys

Have you been invited to an NHS Health Check by your GP in the last two years? If so, Nottinghamshire County Council would love to hear from you, even if you didn’t actually go. We have a short survey for you to understand the experiences of patients to make sure everyone who wants an NHS Health Check can get one.
