The Integrated Child & Family Health Team, is based at Rainworth Medical Centre and comprises a Specialist Practitioner in School Nursing, a Specialist Practitioner in Health Visiting, a Community Health nurse, a Community Health Assistant Practitioner and a Community Health Advisor. The Child & Family Health Team deliver both the universal Healthy Child Programme and targeted Health services for children, young people and their families in the 0-19 age range. Specifically the School Nursing Team offer support and advice to children and their families aged 5-19 years attending the Minster Family of Schools and children who attend independent schools who are registered at the Southwell Medical Centre. Services delivered include: Delivery of the Healthy Child Programme as part of the core offer of services; Vaccination and immunisation sessions at Minster School; School Entry Health Assessments for children attending the Minster Family of schools as part of the Health Child Programme and advice and support on wide range of issues which affect the health and well being of the child (eg. Development, behaviour, minor illness, night time wetting or bereavement). An enuresis clinic operates once a month. The Health Visitor offers support and advice to children and their families aged 0-5 years registered with the Southwell Medical Centre practice. They deliver the Healthy Child Program as part of a universal core service and offer targeted advice and support for children and families dependent on specific needs. Services delivered include: Health and development checks, support on parenting issues, breast-feeding, weaning, postnatal depression, minor illness, complex health issues, speech and language development, behaviour and sleep. |
